If you have not logged on before, you will need to have your Tax ID. You will enter your name and email address by clicking on Provider Access Request on the next screen by clicking here. You will receive an email back from Acuity with your login. If you do not receive an email within 24 hours, please check your spam or junk mail.
If you have not logged in before and your Tax ID is not recognized, please send an email to email@example.com and include the patient’s name and/or group that you are inquiring on along with your W-9. Before your Tax ID can be added to our system, we need to make sure you are treating one of our members.
